Expression of Interest (Updated April 22, 2022) ဆုံမှတ်နိုင်ငံမှတစ်ဆင့် မြန်မာနိုင်ငံသို့ အထူးလေယာဉ်ခရီးစဉ်ဖြင့် ပြန်လည်သွားရောက်လိုသူများ ဖြည့်စွက်ရမည့်ပုံစံ Registration form for Expression of interest (Relief Flight to Myanmar) ကျွန်တော်/ ကျွန်မသည် အမေရိကန်ပြည်ထောင်စုသို့ ရောက်ရှိခဲ့ပြီး သက်ဆိုင်ရာဆုံမှတ်နိုင်ငံမှတစ်ဆင့် အထူးလေယာဉ် (Relief Flight) အစီအစဉ်ဖြင့် မြန်မာနိုင်ငံသို့ ပြန်ရန်ဆန္ဒရှိပါသည်။ ကျွန်တော်/ ကျွန်မအနေဖြင့် မြန်မာနိုင်ငံသို့ ပြန်လည်ရောက်ရှိသည့်အခါ COVID-19 ကာကွယ်ဆေးထိုးပြီးကြောင်း အထောက်အထားပြသနိုင်ပါက Community Based Facility Quarantine (သို့မဟုတ်) သတ်မှတ်ထားသည့် Hotel Quarantine တွင် (၅) ရက် နေထိုင်ရန် လိုအပ်မည်ဖြစ်ပြီး၊ ကာကွယ်ဆေးမထိုးရသေးပါက Community Based Facility Quarantine/ သတ်မှတ်ထားသည့် Hotel Quarantine တွင် (၇) ရက် နေထိုင်ရန် လိုအပ်မည်ဆိုသည့် အချက်ကိုလည်း သိရှိလိုက်နာမည်ဖြစ်ပါသည်။ I hereby declare that I have arrived in the United States and I wish to return to Myanmar with the Relief Flight program via the focal destination country. I acknowledge that I will be required to stay in the mandatory community based facility quarantine or designated hotel quarantine for (5) days if I can present the COVID-19 vaccination card upon arrival in Myanmar. If I am not vaccinated, I will be required to stay in the mandatory community based facility quarantine/ designated hotel quarantine for (7) days upon arrival in Myanmar. ဤပုံစံနှင့်အတူ လျှောက်ထားသူ၏ နိုင်ငံကူးလက်မှတ် (ဓာတ်ပုံပါစာမျက်နှာ)၊ အမေရိကန် ပြည်ဝင်ဗီဇာ၊ အမြဲတမ်းနေထိုင်ခွင့်ရရှိထားသူဖြစ်ပါက Green Card ၊ဆိုက်ရောက် ပြည်ဝင်ဗီဇာ၊ endorsement ပါရှိသည့် စာမျက်နှာတို့ကို ပူးတွဲပေးပို့ရပါမည်။ Please make sure to submit scanned copies of >>>> (1) Passport photo page (2) US / Myanmar visa (3) Arrival stamp by US Immigration (4) Green card copy if applicable.
